Crossroads has a unique referral and application process. Anyone can recommend a person for membership at Crossroads (self, family member, healthcare worker, psychiatrist, Inpatient Services, Day Centre, etc). In order to be eligible for membership, however, the following criteria must be met.

The person:

• is between the ages of 19 and 65
• has been diagnosed with a persistent thought, mood or anxiety disorder
• has a primary therapist within Mental Health Services or a psychiatrist
• has had contact with Mental Health Services
• experiences difficulty in daily life due to his/her mental illness
• is able to work safely within the Clubhouse environment

We encourage the individual to take the first step by going on a tour of Crossroads and completing an application form.

Screening Form

A psychiatrist or a primary therapist in Mental Health Services must complete a screening form if the person is to be considered for membership. Once the person has had a tour of Crossroads, a screening form is delivered to the primary therapist or psychiatrist by the person or mailed by the club.

After completing the screening form, the best way to return it to us is by mail. If you prefer you use fax, please phone us in advance at 567-7047, as our fax machine is not in a private location.

We will contact you if we have any questions during the intake process. If the above criteria are met, the applicant will be invited to participate in our next orientation session.
